2026 Community Champion Nomination Form

50 Forward Mohawk Valley’s Community Champion Award recognizes an individual age 50 or older, who has made a meaningful impact in our community through volunteerism. This award honors someone who has worked to create age-friendly communities, supported positive community change, and made a difference in the lives of older adults through their service. Nominees should have demonstrated a commitment to volunteering with or on behalf of older adults at some point in their service, whether through direct interaction, advocacy, programming, or community-based initiatives.
